Prepare a business case for a trial of an alternative, patient-centric model of primary care adjacent to a residential aged care (RAC) and retirement village (RV) operation in the Mid-North Coast of NSW.
Key Deliverables:
For this Mid-North Coast location:
- Develop an understanding of how GP services are delivered into the RAC/RV complex today – how many GPs, their revenues
- Formulate a “strawman” resident-centric care model e.g. using a patient-centric case management approach
- Identify additional revenues and costs for that strawman resident-centric model for each of the parties
- Articulate how the model might be implemented
– GPsWSI RAC – who, how they would operate e.g. rent-in-kind
– Nurse practitioners
– Physiotherapists
– Real estate – what is required, how funded
– Resident Marketing
Skills and experience required:
- In-depth understanding of primary care and roles of GPs, RNs, NPs, Allied health, pharma etc especially in a geriatric/aged care context
- Detailed understanding of Medicare and Health fund reimbursement categories
- Ability to synthesize financial modelling for each relevant party.
